This is Dr Mangal. Welcome to Fertility and Endometriosis Institute. Please fill out the below new patient history form ASAP. This will give me significant information about your symptoms of endometriosis, pelvic pain and/or infertility.
Please remember to fill out the last page (page 10) in your own words regarding your symptoms since teenage years (chronologically) which led you to this consult with us. The more the detailed, the better I can be prepared to help you. If you need additional space to type, please feel free to do so.
I like to review and know as much information as possible regarding your symptoms and any prior surgeries and treatments for endometriosis/pelvic pain and/or infertility. Then at your consult, I can spend more time with you in the clinic to discuss treatments that can maximize improvement in your quality of life related to endometriosis/pelvic pain and/or infertility.
I will do everything possible to expedite reviewing your history after-hour or the weekend. Then the office will call you in two to three weeks with an appointment date for the type of appointment consultation you’re requesting (Endometriosis/ Pelvic pain and/or Infertility).
